Stedet er Argentina og året er 1960. Ved en tilfeldighet blir den tyske doktor Helmut Gregor oppmerksom på lille blonde Lillith mens hun leker på stranden. Doktoren fatter en helt spesiell interesse for den unge jenta som han omtaler som "et perfekt eksemplar". Gregor tar inn på hotellet Lilliths foreldre driver, og blir etter hvert familiens venn og lege. Men når det kommer for en dag at doktorens virkelige navn er Josef Mengele, snus livene deres på hodet.
